Advance Health Care Directives

An Advance Health Care Directive is an important set of documents that serve to clarify your wishes concerning medical and end of life care, in the event that you are unable to communicate them for yourself. We have all heard and read stories about families that had to make hard decisions on behalf of a loved one going through a health crisis. These are situations that are not easy. How do you know what they would have wanted? With an advance healthcare directive, guesswork is taken out of the picture. In it, you can map out exactly what you do and do not want, should your family find themselves in that situation.

We will walk you through the process of completing your documents. As document preparers, we cannot provide you with medical or legal advice, but we can make sure that they are filled out properly and notarized. We also recommend that you speak to your doctor if you have any questions about any of the medical procedures listed on this paperwork.

We will assist you in preparing and filing the following documents:

Health Care Power of Attorney

If you were to become incapacitated, you can select someone to step in and make healthcare decisions on your behalf. This document allows you to make decisions now regarding treatments that you do and do not want your power of attorney to consent to. This form is used in conjunction with your living will, which lays out your end of life plan for your power of attorney to follow.

Living Will

The living will is a document that allows you to lay out your preference concerning end of life care. To what extent do you want your life prolonged? It addresses topics such as artificially administered foods and fluids, pregnancy concerns, and comfort care. A copy of this form should be given to the person who holds power of attorney.

HIPAA Waivers

These give healthcare workers permission to share health information, should the need arise, with the person/people that you designate.

Mental Health Care Power of Attorney (if desired)

This document is similar to a Health Care Power of Attorney, but specific to mental health care concerns, in terms of incapacity and treatment. 

AZ Healthcare Directive Registry (if desired)

Once the forms are completed, we will submit them, should you choose, to Arizona’s Healthcare Directive Registry, through Health Current. Once they receive your documents, they will send you a wallet card and information as to how to set up an account to access your file. Once that is complete, you can fill out your wallet card with your credentials so that your loved ones and health care providers can access the information if necessary.
